Are you looking for way to assist a Senior in remaining independent, at home, and in the community? Concordia Care, the Cuyahoga County Program for the All-inclusive Care of the Elderly (PACE) is an alternative to nursing home care. Our Interdisciplinary Team of professionals, in partnership with loved ones, provide geriatric support and services necessary for older adults to safely remain in their homes. We are the only program of our kind in Northeast Ohio. 

In Cuyahoga County, 26% of individuals age 60+ consider themselves to be in fair or poor health.
